Provides professional initial and ongoing assessment to an adult population of patients with SCI in the outpatient clinical telehealth setting. Work is guided by SCI clinical practice guidelines, supervisory instructions and communication with the interdisciplinary team. The SCI Telehealth Nurse Care Coordinator utilizes telehealth/virtual care technologies to apply clinical process skills and comprehensive assessments and care planning processes for persons with SCI. Demonstrates knowledge of clinical issues specific to the SCI population, injury level-related issues as well as changes associated with the aging process. Maintains knowledge about disease processes, disabilities, medications and their clinical sequelae. Accesses and interprets data about the patient's social, emotional, mental health, medical needs and coordinate the care/services needed. Provides assistance, information and support to patients and their families in coping with emotional, practical and lifestyle issues, which accompany SCI, advancing age and physical sensory and cognitive impairments. Knowledge of the vast array of Veteran Administration entitlements and benefits, local community agencies and resources, and how to access and coordinate these services. In the medical facility setting, focuses on communication of patient care in the context of the Veteran Health Administration (VHA) SCI Hub and Spokes System of Care, coordinating referrals for specialty tele-consultations, post-discharge follow-up telehealth visits with the SCI Center clinical team, Disease Management Programs and specialty consultation to home telehealth patient education, issuance of home telehealth equipment, secure messaging and other virtual technologies for clinical and educational purposes. Responsible for coordinating the appropriate intensity of care management for his/her panel. Care management is focused primarily on providing more coordinated and higher quality care to complex clinical cases with a goal of preventing debilitating clinical complications and reducing the need for inpatient admissions. Assess telehealth patient self-management skills and provide patient education/intervention to enhance these skills. Arranges, coordinates, monitors, evaluates and advocates for a package of multiple services, including use of telehealth/virtual care technologies and services to meet the patient's complex needs. Documents all routine and non-routine screening, assessments, interventions, education, referrals in CPRS provided to telehealth patients, using appropriate templates and codes as outlined by VA telehealth program policies and directives. Completes clinical reminders. Develops and administers regional SCI hub and spokes telehealth program, through collaboration with the interdisciplinary teams at hum and spokes in concordance with VHA Directive 1176, Spinal Cord Injury and Disorders System of Care. Develop, implement and articulate new telehealth program initiatives. Serves as technical telehealth consultant for all aspects of program telehealth use, including training, pilot testing and implementing new long distance care models, multi-site coordination and problem resolution. Serves as liaison and encourages staff from SCI Center and SCI spoke sites to adopt innovative clinical practices and refer patients to appropriate telehealth/virtual care programs. Identifies, coordinates and provides training for existing and emerging telehealth/virtual care technologies. Acts as resource person in planning, reviewing or revising policies and procedures that impact clinical provider training and patient care in telehealth settings. Ensures compliance with requirements of internal and external accreditation organizations. Helps prepare for CoP and other review processes. Establishes methods for tracking patients' progress, evaluating effectiveness of care and maintains appropriate documentation of patients' care and progress within the plan. Develops, implements and maintains processes and mechanism for scheduling telehealth visits between patient/provider or hub/spoke sites. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:

The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. The check will bounce and the victim is left responsible.
