Nurse Practitioner - Home Based Primary Care

Home Based Primary Care (HBPC) provides care in the home for patients with acute and chronic health conditions or change in mental status; these conditions negatively influence normal individual functioning. Patients served are all ages, but are primarily elderly. All have complex medical problems with multiple diagnoses, including diabetes, CHF, COPD, cancer and neurological problems such as multiple sclerosis. Duties include but are not limited to:
observe and interpret patterns of patient behavior and/or problems in operational systems or processes; discriminates complex patient/family symptoms and needs and/or system issues during assessments; identifies intricate patient/systems problems and accurately focuses on those problem areas provide age-specific care for adults of all ages, particularly the older adult (65-74) and geriatric (over 75), which includes the following elements related to the process and/or affects of chronic illness:
promotes independence, encourages self-care; involves family or designated caregiver with care; provides any necessary safety measures, and assists patient to cope with and/or compensate for changes affecting mobility, balance, loss of cognition, eyesight, hearing, bones, skin Assesses age-specific care requirements, cultural religious, and gender variables and develops appropriate plan of care investigate health problems or health needs by gathering and assessing historical information perform physical examinations diagnose problems based on interpretation of the patient's history, physical exam findings and diagnostic test results develop a treatment plan tailored to the patient's needs, which includes health education, pharmacologic and non-pharmacologic treatment prescribe medications as appropriate evaluate the results of the treatment plan, modifying the plan as needed to achieve treatment goals make referrals for additional specialty evaluation or treatment work collaboratively with a multidisciplinary team in the care of the patient manage a panel of patients in their homes, spread over geographic area within a 30 mile radius of the home facility Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Experience:
Minimum 2 years experience working with geriatric population required. Home-based Primary Care experience preferred. Nurse Practitioners:
A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Reference:

Heavy lifting (45 lbs), straight pulling (6 hrs), pushing (2 hrs), use of fingers, walking (6 hrs), standing (2 hrs), repeated bending (6 hrs), ability to distinguish basic colors, and hearing (aid permitted).

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
