Registered Nurse - Patient Access Coordinator

As Nurse Coordinator, job duties, tasks and responsibilities include but are not limited to:
Meets the needs of customers while supporting VA missions including ICARE values; consistently communicates and treats internal and external customers in a courteous, tactful and respectful manner Documents the condition, progress and plan of care of patients in acute community hospital beds, daily FEE (VA pay) patients, as needed for all hospitalized Veterans Coordinates patient transfers and clinical consults/referral services for inpatient and specialty care between VA and non-VA facilities; evaluates patient's stability, required level of care and other critical needs when considering transfer to VAPSCHS from the community; prioritizes potential transfers based on local and national policies, regulations and directives when beds are available Provides alerts and notifications for other clinical and social work patient care team members as needed to complete their plan of care and assist in follow up care Provides timely and appropriate discharge planning coordination with community partners and VA clinicians; works daily with community care managers, social workers, providers and nurses to ensure Veterans have an appropriate and safe plan of care, including VA services when applicable and appropriate. Duties when assigned to the specific role of VISN Integrated Referral Service (VIRS) and Traveling Veteran Program include the following:
coordinates care for Veterans traveling to and/or from other VAs who require services from VAPSCHS Provides guidance to the VAPSHCS providers regarding the transfer and referral process, facilitating the delivery of quality care, as directed by local, VISN and national regulations and policies Manages inter-facility consults, lodging and travel requests and Traveling Veteran self-referrals timely and accurately Collaborates with providers and other clinicians and implements the patient plan of care related to patient transfers and clinical/referral requests Maintains tracking tools; collects, analyzes and reports data and trends to assure safe, effective and appropriate patient care delivery Provides non-supervisory oversight of the TVC and VIRS programs Educates Veterans, their families and staff regarding the processes and POC information for access to care through both programs. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Bachelor of Science in Nursing 2-3 year successful nursing practice Previous care coordination experience Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Heavy lifting (45 lbs. and over), straight pulling (6 hrs.), pushing (2 hrs.), reaching above shoulder, use of fingers, walking (6 hrs.), standing (2 hrs.), repeated bending (6 hrs.), ability to distinguish basic colors, hearing (aid permitted).

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
